I took Affinity Health- it was explained to me that all pathology test are cover aswell as all radiologist accounts we must just know it is only black and white xtrays. I had pain in my leg so i went to the doctor.The Dr did X-Rays at Tshepo Themba Clinix hospital. I then recieved two statements from the Pathology Dr and the Radiology saying tht i have to pay their monies as Affinity refused to pay and they said Affility said they dont cover such test(blood test and X-Rays). So please tell me what is an X-Ray is it not radiologist? What is a bloodtest - is it not pathology? I am no doctor but seriously.
Kindly assist with this matter.
I would like to take this opportunity to complain about a product that I personally think where created to pray on people.
My daughter have been a member since May of 2013.
On 14 July 2014 at 16H00 we had to rush my daughter to Casualties at Union Hospital in Alberton with a possible Appendicitis. SHE WAS IN SEVERE PAIN.
The staff at Union was send from heaven and L*** was made to feel comfortable in no time.
Then the bad news. Affinity was phoned for authorisation for test and admitting her. Well well after more than a year of faithful payments we were told that blood test, sonars and x-rays are for our own account. We had to pay the Hospital and doctor first before the doctor could assist. R2080.
Affinity Heath will admit her but only R11 000 for two days. Yet her membership document states that L**** qualifies for R30 000 for Appendicitis. After lots of stress and worries L*** was transferred to Natalspruit Hospital for the emergency operation. This was 03H00 in the morning. This means L*** was in pain for 11 hours just because Affinity did not want to assist.

I would like to clarify that is for Affinity Insurance and not Affinity Health.I see that Affinity Insurance does not respond to any listings on this site and confirmed this morning that Affinity Health and Affinity Insurance are the same company,different divisions.Being in an accident is a traumatic experience especially when it's due to someone else's negligence.You are supposed to have some comfort in the fact that you are insured.My experience with Affinity Insurance has been nothing more than absolutely disgusting and reading the other listings i see I am not the only person.Claim forms was sent to them on the - 31/05/2014On the 24/06/2014 they requested a settlement letter I sent them the settlement letter and the registration certificate on the 25/06/2014They sent me the first agreement on the 09/07/2014 however they were charging me a penalty of R 23 880 for not submitting the inspection certificate , however this document was sent to them when I took the insurance , they will not take an insurance without this. Anyway they insisted that I didn't send it so I got another copy and sent it.This charge was waivered.I was entitled to R7500 which has not yet paid out.

Dominique from the telesales sold me the accident plan I have been paying for months but have not yet received my welcome package by courier as promissed with my medical card contract stating everything and should I now experence an emergency I have NO PROOF I HAVE COVER!

The guy told me that I can have health insurance and medical aid and that they pay out for the hospital part of the policy R6500 for the first day R4500 for day 2-3 and R3000 for day 4-5 and after that R1500 up to 21 days. My child was in hospital at the beginning of July 2014 with Bronco phneumonia. Now they say no my medical aid paid the hospital bill so they are not going to pay anything. In there FAQ they say that the benefits would be paid into my account and that I would be responsible for settling the hospital. Chantelle told me that there would be no pay out to me because Discovery paid. No where in the documents that I received is stated that they would pay the balance of what Discovery did not pay for my hospital benefit. Op die 24ste Junie het ek gegly en geval ek was nog in die 30 dae wag periode aangesien ek die 1ste Junie 2014 aangesluit het. Wat ek nie verstaan het is dat ek die ongeluksvoordeel kon gebruik het maar die oproep sentrum het nie eers 'n aantekening gemaak dat ek gebel het die 24ste Junie en navraag gedoen het wat om te doen nie. Intussen is dit al vandag die 21ste Augustus my enkel het nog nie aangegroei nie ek het die Drs verslag gestuur om die Ortopeet te sien maar ek het vandag self die Ortopeet betaal. Die Ortopeet het 'n MR scan versoek aangesien hy nie op die plate alles kan sien nie waar ek uitgevind het Affinity dek geen scans nie net x-strale. Chantal Boylan by die call centre is die enigste hulp sover. Dit is kommerwekkend dat mens hierdie valse hoop en vertroosting het om te dink jy het dekking as daar iets gebeur maar dit is nie waar nie. Ek was ook meegedeel enige hospitaal maar dan kom jy by die hospitaal dan behandel hulle jou as 'n privaat pasient. Jy het hierdie wanpersepsie oor die goedkoper premie maar intussen betaal jy baie meer jy kan net sowel 'n ordentlike mediese fonds uitneem wat alles dek as om so baie bybetalings te he.

I came to Alaska in 1988, following my parent up here and fell in love with Seward. Since then I have felt proud to be a part of Alaska and Seward. I have served on the Seward Fire Department for almost 25 years and currently hold the rank of Captain. I have served with the Volunteer Ambulance Corps, I am currently the president of the AVTEC teachers union, A Commissioner for Planning and Zoning and I helped form and coach the Seward Seahawks football team when I can.
I love Seward and feel that we are on the brink of tremendous growth in Seward. With all the growth in our port for cargo , fuel, and SMIC we are looking at becoming the gateway to Alaska. Because of my experience in the maritime field and my service for the city so far I believe I am the best candidate for City Council. My name is Marianna Keil and I have been the Seward Legislative Information Officer since 1993. I worked as the Domestic Violence Sexual Assault Coordinator during the interim and served on the DVSA Advisory Committee for many years, appointed to the Seward Planning and Zoning Commission for seven years and I was the President of the Seward 2003 Centennial Committee. All of these positions have given me great appreciation for our community. When people ask me why I live in Seward, I tell them it is a safe place to raise a family, with wonderful people who look out for each other and our town has the most beautiful scenery in the world.
I have served on the Seward City Council since 2008. During my tenure, the city has weathered the economic downturn and has seen an improved financial position, received legislative appropriations for the Seward Marine Industrial Center, airport improvements and Seward sewer lagoon sludge removal, completion of the Seward Library Museum and Providence Seward Mountain Haven, Seward harbor dock improvements and grants for the Seward Community Health Center. I would like to remain on council to see the Electrical building completed and upgrades to the electrical system, the completion of the harbor dock renovations, start of the airport and Alaska Railroad Dock improvements, removal of the sludge in the sewer lagoon, the start of the SMIC breakwater, the home porting of the research vessel Sikuliaq, road and sidewalk improvements and continue the progress towards stable year round employment for Seward's citizens. Not only have many things been accomplished but there also are great things to come.
The current council is working well together and every member has the well-being of the community at heart. There will always be differences of opinions, which leads to healthy discussions, but when the meeting ends everyone leaves feeling that they have advocated their side of the issues. The public is very important to the process and there are positions open on the Seward Planning and Zoning Commission and the Historical Preservation Commission. I would hope that the volunteerism so prevalent in Seward will extend to these important city boards.
I believe that I possess a calm nature, common sense, experience in budgets and policy and the ability to work well with the public, my peers and staff.

My ancestors instilled in me a strong sense of place and the importance of service above self. My mother, the daughter of a missionary, was raised in China between the great wars. After college but before graduate school I taught as a Peace Corps Volunteer in Malaysia. I have lived in Seward for the last 16 years, which is longer than I have lived anywhere else. So Seward is my place and serving on the City Council would meet my continuing commitment to serving my community.
I am a scientist and an educator. However, unlike many I do not focus solely on the area of my training. I seek the knowledge and information most applicable to the problems I am trying to solve. As a result, although my graduate training was in molecular and environmental toxicology I have gained nationally recognized expectise in public health, seafood technology, fisheries and marine sciences as well as toxicology. As one of my early academic advisors told me, if you get a strong basic education you can apply it where ever the need takes you. You can be sure I will do my homework on the City Council.
Over the 34 years I have lived in Alaska I have served in many governmental and quasi-governmental roles In Kodiak, I was elected to the Chamber of Commerce Board of Directors, and served as Co-Chair of the Near Island Development Authority. Near Island is the City of Kodiak's SMIC. Once the harbor was completed and the island was connected to the City by a bridge, the City of Kodiak needed a plan to develop the uplands. However the "Authority" was really just an advisory committee. I served on the first public advisory group for the Exxon Valdez Oil Spill Trustee Council, and continue to serve having served six two year terms since 1991.
Since moving to Seward, I served 13 years representing the City on Cook Inlet Aquaculture Association's Board of Directors, 10 years on the Prince William Sound Regional Citizens' Advisory Council Board of Directors where I also served as the Chair of its Oil Spill Prevention and Response Committee and the Scientific Advisory Committee. I have also volunteered many hours in a variety of roles at the Alaska SeaLife Center, for the Seward Silver Salmon Derby, and other community and professional organizations.
I currently serve as the Vice-Chair of the Historic Preservation Commission. Although I do not consider myself a historian, rather I am a student of history. I feel it is very important to understand history on all levels from locally to globally, so we can build upon successes and avoid the pitfalls of failures.
We
should embrace Seward's diversity and respect all our fellow residents.
My goal is to represent all of Seward, not just those I feel the
greatest affinity with. City government should be open and transparent.
Both procurement and sale of assets should be on an open competitive
basis. The appearance of a conflict of interest is often just as
detrimental as an actual monetary conflict

UWO alumnus achieves a healthy dose of career success
For University of Wisconsin Oshkosh Thomas Zoch ’79, of Neenah, the
time spent as an undergraduate at UW Oshkosh gave him the foundation he
needed to start his medical career and was the home plate for his
journey.
His success in the medical field and his leadership of the Network Health HouseCalls program has made him one of this year’s recipients of the 2014 Alumni Association Distinguished Alumni Award.
As a student at UW Oshkosh, Zoch balanced his time between sports and academics. He played on the basketball team from 1974 to 1975, and on the baseball team from 1975 to 1976 and again from 1978 to 1979.
Zoch made the Wisconsin State University Conference (WSUC) All-Conference in basketball in 1979 and the WSUC Scholastic Honor Roll three times.
Academically, Zoch also was highly successful. He made the Dean’s List every year and was named the Biology Major of the Year in 1979. He graduated magna cum laude with bachelor’s degrees in biology and chemistry.
Immediately following graduation, Zoch went to medical school at the Medical College of Wisconsin and earned his master’s degree. He then completed his internal medicine residency at Marshfield Clinic and achieved board certification in internal medicine in 1986.
He worked as a staff physician at the Marshfield Clinic until 1995. During his time there, he helped develop the clinic’s emergency department, urgent care department and sports medicine clinic.
As the emergency department education director, he was responsible for teaching surgery, internal medicine, pediatric, dermatology and transitional residents, medical students, EMT and paramedic students, physician assistants and nurse practitioners.
In 1995, Zoch began working as a staff emergency physician at Theda Clark Regional Medical Center. He was the medical director of the Theda Clark Regional Medical Center Emergency Department and the president of Fox Valley Emergency Medicine from 1997 to 2003.
Zoch also served as the team physician for the Timber Rattlers from 1995 to 2005, and now serves as one of their associate board members. He became a staff physician with ThedaCare and joined their occupational medicine department from 2003 to 2005, and then their internal medicine department from 2005 to 2008, before joining the ThedaCare orthopedics plus as a sports medicine specialist until 2010.
In 2010, Zoch was recruited by Affinity Health System’s Network Health as their associate medical director. He has since completed his certification as a Healthcare Insurance Executive (CHIE) and works in this position today. He also is the medical director for Network Housecalls.
The HouseCalls program provides in-home health assessments to Network Health Medicare Advantage members. The program sends licensed HouseCalls healthcare professionals to members’ homes, where they complete a review of the patient’s health history, current health status and any health concerns.
“Dr. Zoch has led this program since its launch in Oct. 2011,” Network Health President Sheila Jenkins said. “More than 4,000 exams have been completed and the program continues to grow.”
Zoch’s vision has helped the program provide in-home exams and healthcare for more than 10,000 patients as of 2013.
In addition, Zoch is the executive leader of Ministry Health Care, the medical director for the Fox Valley Technical College Paramedic/EMS program and a board member for Ministry Home Care.
Zoch has been involved in the American Heart Association (AHA). In 2002, he received the Dr. Dean Stueland AHA Award for his dedication to Advanced Cardiac Life Support leadership. He led the National AHA First Aid Guidelines for 2005, received the AHA Above and Beyond recognition award and the Certificate of Appreciation from the National First Aid Science Advisory Board. He also was part of the AHA National Faculty for Advanced Cardiac Life Support, where he represented Wisconsin for four years.
In addition to his extensive involvement in the healthcare community, Zoch has continued to play baseball for the Wisconsin State League, a semi-professional baseball league, in Marshfield, Green Bay and Oshkosh. He was inducted into the Wisconsin State Baseball League Hall of Fame and the Marshfield’s Baseball Hall of Fame.
His athletic ability has been recognized by his coach, Tom Kraus.
“He has demonstrated outstanding character and wisdom throughout his tenure as a baseball player,” Kraus said. “Not only did he use his God-given talent as a greater player, he also made decisions which have done more for the well-being of mankind.”
Despite experiencing an athletic injury early in his college career that affected his confidence in further college athletic participation, he learned a valuable lesson about pursuing and playing the sport that he loves.
“Life has a consistent way of knocking you down, but what really matters is how you get up and move forward,” Zoch said.
Zoch’s success in the medical field has been a home run, but he remembers his time at UW Oshkosh as the foundation for his achievements and accomplishments in healthcare.
“UWO gave me a great start on advanced education and provided many educational opportunities,” Zoch said. “Very few individuals in my extended family were granted these opportunities. Therefore, I have never taken these opportunities for granted and I feel very blessed and deeply appreciative of the education I received at UWO.”
Zoch and nine other award winners will be recognized at the Alumni Award Celebration during Homecoming 2014 on Friday, Oct. 17.
On the 4th of February 2014 I completed the "Get a Quote" section of the Affinity Health website as I was interested in finding out more about their scheme. Shortly thereafter I receive a Medical Questionnaire which I did not complete.On the 11th of February I received a Document Pack and a notification that I had been "successfully accepted" as a member and that the premium was R 1518. I wrote to the supplied address stating that I did not agree to the membership and that I wanted it cancelled immediately. I received a reply from Jackie van Niekerk who stated that there was a 30 day cancellation period and that I would be debited. My account has been debited twice with R 1518 and also R 150 all by Affinity Health. Due to insufficient funds the unauthorized debits were returned incurring charges of R 360. To the Director of Affinity Health, Mr Murray Hewlett and the General manager Mr Mc Namee I find your company's conduct disgusting and underhanded. His success in the medical field and his leadership of the Network Health HouseCalls program has made him one of this year’s recipients of the 2014 Alumni Association Distinguished Alumni Award.
As a student at UW Oshkosh, Zoch balanced his time between sports and academics. He played on the basketball team from 1974 to 1975, and on the baseball team from 1975 to 1976 and again from 1978 to 1979.
Zoch made the Wisconsin State University Conference (WSUC) All-Conference in basketball in 1979 and the WSUC Scholastic Honor Roll three times.
Academically, Zoch also was highly successful. He made the Dean’s List every year and was named the Biology Major of the Year in 1979. He graduated magna cum laude with bachelor’s degrees in biology and chemistry.
Immediately following graduation, Zoch went to medical school at the Medical College of Wisconsin and earned his master’s degree. He then completed his internal medicine residency at Marshfield Clinic and achieved board certification in internal medicine in 1986.
He worked as a staff physician at the Marshfield Clinic until 1995. During his time there, he helped develop the clinic’s emergency department, urgent care department and sports medicine clinic.
As the emergency department education director, he was responsible for teaching surgery, internal medicine, pediatric, dermatology and transitional residents, medical students, EMT and paramedic students, physician assistants and nurse practitioners.
In 1995, Zoch began working as a staff emergency physician at Theda Clark Regional Medical Center. He was the medical director of the Theda Clark Regional Medical Center Emergency Department and the president of Fox Valley Emergency Medicine from 1997 to 2003.
Zoch also served as the team physician for the Timber Rattlers from 1995 to 2005, and now serves as one of their associate board members. He became a staff physician with ThedaCare and joined their occupational medicine department from 2003 to 2005, and then their internal medicine department from 2005 to 2008, before joining the ThedaCare orthopedics plus as a sports medicine specialist until 2010.
